Columbia, Greene, and Dutchess Counties’ Network for Care 


For residents and visitors in Columbia, Greene, and northern Dutchess counties, there’s one name they can always turn to for their healthcare needs: Columbia Memorial Health (CMH). Over the past 20 years, CMH has grown from an acute-care hospital in Hudson to a total of 40 care centers across three counties that provide a diverse range of primary, pediatric, emergency, and specialty services. “Our growth has been a result of a vision to better serve the community,” says Dr. Ronald Pope, Vice President of Medical Services, Care Centers for CMH. 

Expanding Access in Rural Communities 

As the leading healthcare center for Columbia and Greene counties, expanding access to services for traditionally underserved communities in rural areas has always been a high priority for CMH. With family medicine practices in Copake, Valatie, Coxsackie, and Windham, as well as Rapid Care emergency walk-in centers in Copake and Valatie, families can easily find a provider nearby to oversee their loved ones’ primary, pediatric, and emergency care. 

Comprehensive Team-Based Care

To increase collaboration among its providers and provide a more comprehensive care plan for patients with intensive needs, CMH’s primary care doctors work in an innovative team-based model. Providers work closely with specialized nurse case managers, care coordinators, behaviorists, pharmacists, and psychiatrists to troubleshoot important issues that can affect patient health outcomes, like financial, transportation, or mental health challenges. 

“In the old model, the physician was responsible for everything, but they didn’t have time to address everything the patient was going through,” says Dr. Pope. “Now our providers get to realize the power of a whole team working together on one patient, and our patients feel better supported, too.”  

Community-Centered Specialty Services

Thanks in part to its affiliation with Albany Med, specialty care services have remained one of CMH’s strengths. With specialty practices centered in Hudson and Catskill, including women’s health, neurosurgery, urology, gastroenterology, orthopedics, cardiology, general and colorectal surgery, and pain management—plus access to Albany Med’s team of specialists—patients in Columbia, Greene, and northern Dutchess counties have an accessible, world-class network of care.
